This template will display the Contact us form.

Step 1 : Log into your WordPress Dashboard and navigate to Pages > Add New

Step 2 : Locate the Template and in the dropdown menu, select Contact.

Step 3 : Banner Options

Choose Banner Type : Choose to display a map or use the default Page / Post Header Options

Address for Map : Insert your address for the Google map.

Step 4 : Contact Options

Contact Email : Insert an Email Address for your contact us page.

Step 5 : When you are finished, Update your page, it should looks like this.

Step 6 : Select your preferred sidebar in the dropdown list and layout for the sidebar and content.
